Who does this apply to:
- Patients seen in nurse consultation with raised BP readings.
- GPs can refer for further assessment of raised blood pressure reading – ask patient to make appointment with Kay/Cally for blood pressure monitoring.
The protocol does not apply to:
- Patients with diagnosed hypertension
All investigations to be coded by “O/E- BP reading raised” (using QOF code)
Diagnosis of Hypertension to be added by GP

Who does this apply to:
- Patients requiring home blood pressure monitoring:
– Persistently raised readings (see Hypertension diagnosis protocol)
– Referred by GP / Chronic Disease Nurse- Jackie or Jayne
Protocol for use of Waiting Room automatic machine
- Who does this apply to?
- Patients who have been invited by text message to have a BP check- either because they already have a hypertension diagnosis and have not had a reading recorded in the past year-OR because they do not have a diagnosis and have not had a BP reading in the past 5 years.
- This does not apply to patients who have Diabetes
