Purpose
To provide a consistent, sensitive and safe process for managing notifications of patient deaths within Practice Services, including communication with relatives, Medical Examiner and Coroner processes, death registration, record deduction, appointment cancellation and clinical governance.
Scope
This SOP applies to Health Navigators/Receptionists, Admin and Deductions staff, Secretaries, GPs and the After Death team when the practice is notified of a patient death, including deaths in the community, hospital, out of area or abroad.
