Reporting Learning Events

It is important we take opportunities to learn when things go wrong…….

When things go wrong, or could have done, we want to know so we can put it right.

The BrisDoc Learning Event form can be found on the BrisDoc weblinks page as Report Form. The person responsible for filling in these forms is YOU!

Of course you can flag things to the shift manager to help resolve them, but you should also fill in a form we can carry out a proper investigation to help us to continuously improve our processes.

Some things to know about filling in the forms:

  • Every one that is received by the Governance Team is recorded on our system (called LERIS). They are then reviewed by the appropriate manager, who can investigate what happened and see what improvements we need to make
  • Clear actions must be identified and completed before the incident is closed on the system
  • We WANT to receive forms. If problems aren’t reported, we can’t put them right, and if it’s not written down we have no way of properly making sure that things have been followed up
  • We won’t use forms to assign blame, but rather to look carefully as to why a mistake happened. Perhaps we need to provide more training, or change the way we do things
  • Once we’ve identified what went wrong and identified actions, we can feed back to you and let you know

Improving our service means making sure we report things that don’t go well. It’s hard to be precise about what needs to be reported, but if you think someone needs to know – fill in a form.
